When I am lucky enough to have worked with a client for decades and they transition into various phases of life, it’s not uncommon for my role as a Wealth Manager to extend beyond dollars and cents into giving advice on personal matters. 

In those moments, it’s an honor to be a trusted voice they feel comfortable coming to for guidance on matters that transcend my “line of business.”  

One of the most important decisions many adults find themselves facing as their parents age and begin to need higher levels of care is vetting assisted living facilities. This came up for me recently with a client and I was reminded of just how overwhelming it can be to navigate the research, visits, and insurance.  

The stakes are high: there may be a sense of urgency to make a decision and you’d be surprised how many people are going it alone because they don’t know who to ask for unbiased advice. 

What I think everyone in that situation is really looking for is guidance on what you should ask to build confidence you’re making the right decision.  

To help, I compiled a list of 50+ questions you can print and bring with you to any facility visit that cover a wide range of topics, many you might not even be aware of unless you’ve gone through it before.

General Questions for All Facilities 

Facility Information 

  • What is the facility’s accreditation and licensing status? 
  • How long has the facility been in operation? 
  • What is the staff-to-resident ratio? 
  • What are the qualifications and training of the staff? 
  • Are background checks conducted on all staff members? 
  • What is the staff turnover rate? 

Living Environment 

  • Can I see a sample apartment or room? 
  • Are the common areas clean and well-maintained? 
  • Is the facility secure (e.g., locked doors, surveillance cameras)? 
  • What safety measures are in place (e.g., handrails, emergency call systems)? 
  • Is there an outdoor area for residents to use? 
  • Can residents have vehicles? If yes, is there an additional cost? 

Medical and Personal Care 

  • What medical services are available on-site? 
  • How is medication managed? 
  • How are medical emergencies handled? 
  • Are there nurses or doctors on-site or on-call 24/7? 
  • How are personal care plans developed and updated? 

Activities and Social Engagement 

  • What types of activities and events are offered? 
  • Is there a monthly activity calendar? 
  • How are residents encouraged to participate in activities? 
  • Are there opportunities for outings or community involvement? 
  • What is the policy on visitors? 

Food and Nutrition 

  • Can I see a sample menu? 
  • How are special dietary needs accommodated? 
  • Are snacks and beverages available between meals? 
  • Can residents eat in their rooms if they choose? 

Costs and Financials 

  • What is the cost structure (e.g., monthly fees, additional costs)? 
  • What services are included in the base cost? 
  • Are there additional charges for specific services or activities? 
  • What is the policy for rate increases? 
  • What is the policy on deposits and refunds? 

Policies and Procedures 

  • What is the admission and discharge policy? 
  • What are the rules for personal belongings and room decorations? 
  • What is the procedure for handling complaints? 
  • How does the facility handle end-of-life care? 

Specific Questions for Nursing Homes 

Care and Services 

  • What types of rehabilitation services are offered? 
  • Are there specialized programs for residents with dementia or Alzheimer’s? 
  • How often are care plans reviewed and updated? 

Staffing and Training 

  • What is the ratio of certified nursing assistants (CNAs) to residents? 
  • What ongoing training do staff members receive? 

Specific Questions for Assisted Living Facilities 

Assistance and Support 

  • What types of assistance with daily living activities (e.g., bathing, dressing) are provided? 
  • How does the facility promote independence among residents? 
  • What transportation services are available for medical appointments? 

Specific Questions for Independent Living Facilities 

Lifestyle and Independence 

  • What amenities are included (e.g., fitness center, swimming pool)? 
  • Are housekeeping and laundry services provided? 
  • What transportation options are available for shopping and social activities? 

Additional Considerations 

Family and Resident Input 

  • Can I speak with current residents and their families about their experiences? 
  • Are resident and family councils active within the facility? 

Visits and Tours 

  • Can I take an unscheduled tour of the facility? 
  • Are virtual tours available for family members who cannot visit in person? 

Contracts and Agreements 

  • Can I review the residency agreement or contract? 
  • Are there any conditions that would lead to a resident’s discharge? 

Post-Visit Reflection 

  • What was my overall impression of the facility? 
  • Did the staff and residents seem happy and engaged? 
  • Were my questions answered thoroughly and respectfully?  

Additional Questions 

  • How do they handle unexpected situations involving a client/ resident or a staff? 
  • Are there any management or ownership changes expected in the near future?
